The focus is to facilitate patient recovery in the community, with the aim of reducing the need for inpatient treatment, whilst trying to support the patient to remain as independent in their lives as possible. The focus of treatment is to improve physical and mental health with support from a full multidisciplinary team. The service also supports families and carers, and links with other services to help develop safe and effective pathways for patients.

The post will require travelling to various sites, parking in our Trust car parks is free. There are hot desking offices available for Dietitians to use across the Trust and we will provide you with IT equipment to enable you to work across multiple sites and access our electronic notes and dairies as needed.
